  • If bodily injury, name, age and relationship of injured person
  • Extent of injury/person complaining of what?
  • Was the person taken to a hospital?
  • Were there any witnesses?
  • Was a police report made?
  • What is the police report number?
  • Have you been contacted by an attorney representing the injured party?
